A photocopy of your original sales receipt, customer service agreement or other proof of purchase, detailing the date of the handset or device purchase, was not submitted.
The date on your store receipt or proof of purchase was not within the valid dates of the offer.
A rebate card request form was not submitted. An original or copy is required.
A 10-digit T-Mobile wireless number was not submitted.
The 15-digit IMEI (Handset ID) from your product was not submitted or contained alpha-numeric characters.
Your submission was postmarked after the specified last submission date for this offer.
The 10-digit T-Mobile wireless number you provided is currently not showing in our system as an active T-Mobile wireless number.

New Activation
: This rebate card offer requires activation on a T-Mobile wireless calling plan with a minimum monthly charge as specified on your rebate card request form.

Upgrade
: This rebate card offer requires that you currently are on T-Mobile wireless calling plan with a minimum monthly charge as specified on your rebate card request form and upgrade contract length.

New Activation/Pay As You Go (Prepaid)
: This promotion requires that you are a new activation OR added a new line of service (to an existing account). It also requires that the new handset purchase occur between the specified offer dates on the rebate card request form.

Upgrade
: This rebate card offer requires that you purchase your upgrade handset or phone between the eligible dates as listed on the rebate card request form.
This rebate card offer requires that your purchase of a new handset cannot be from Wal-Mart, Target or Sams Club store that provides discounted handsets at the point of sale.
The white sticker label from the side of your handset packaging contains an IMEI (Handset ID) number that has already been used on a rebate card.
The phone number and IMEI (Handset ID) number you provided indicate your T-Mobile account has already received a rebate card related credit adjustment or the handset was provided an instant rebate card at the point of sale.
