Reason:

  • The rebate card program is closed. Mail postmarked after the last submission date on the rebate card request form is returned to sender.

Actions to take:

Open your rebate card submission to verify the correct form was sent for your date of purchase.
  1. Check the purchase date on the proof of purchase (receipt) submitted for the rebate card.
  2. Compare the date of purchase to the rebate card offer dates- located on the rebate card request form.
    • For New Activation/Pay As You Go (Prepaid) rebate cards: Located in the first bullet point under CONDITIONS
    • For Upgrade rebate cards: Located on the rebate card request form in the section titled Upgrade Fulfillment Must Be As Outlined:
  3. If the date of purchase is NOT within the dates on the rebate card request form:
    • Go to T-Mobile Rebate Cards Home Page > and search for the correct rebate card request form, if applicable.
    • Resubmit with the correct rebate card request form (completed) per the instructions on this rebate card request form.
    • Resubmit to the address on the new rebate card request form. DO NOT USE THE ORIGINAL REBATE CARD ADDRESS.
  4. If the date of purchase IS within the dates on the rebate card request form: Per the terms and conditions on the rebate card request form, the rebate card submission MUST be postmarked by the final submission date.
